Requirements
- Registered Nurse (RN) license
- Ability to assess patients’ home environment and caretaker readiness for home dialysis
- Knowledge of nephrology health care team practices, including decision-making, teaching, leadership, and quality improvement
- Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
Responsibilities
- Identify and evaluate candidates for the Home Therapies Program, including assessment of patients’ home environment if needed
- Perform assessment and identify barriers of the Home Dialysis candidate’s home environment and caretaker readiness and ability to perform dialysis treatments in the home
- Admit new patients to the Home Therapies Program using the appropriate processes
- Coordinate patient care including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients
- Assess and manage patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols
- Analyze dialysis data for Home Therapy patients and document and adjust or modify the treatment plan as needed
- Provide initial and ongoing education to patient and family on home dialysis treatment and the operation and maintenance of all home therapies equipment
- Ensure patient awareness related to transplant and treatment modality options
- Administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness
- Initiate or assist with emergency response measures
- Complete routine patient evaluations within the clinic setting
- Ensure correct laboratory collection, processing, and shipping procedures are performed
- Collaborate with the Interdisciplinary Team
