Requirements

MS in Nursing or Master of Physician Assistant Studies
Current, unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in the state of Indiana
Possession of DEA registration or eligibility
CPR certification
Satisfactory completion of a Tuberculosis test (for Carelon Health business unit)
Reside and be licensed in the state of Indiana
Within reasonable commuting distance from New Castle, Connersville, and Richmond, Indiana (unless accommodation granted)

Responsibilities

Collaborate with company physicians, patient’s other physicians/providers, and family members to develop complex plans of care based on patient’s health status, goals, and values
Provide clinical and non-clinical support to patients in palliative care within New Castle, Connersville, and Richmond, Indiana
Provide services in home settings and via telehealth
Develop and implement clinical plans of care for adult patients with chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited resources, chronic medical conditions)
Gather history, physical exams, and diagnostics as needed, and develop/implement treatment plans based on patient’s goals and conditions
Meet patient’s and family’s physical and psychosocial needs with support from inter-disciplinary team
Educate patients and families on medication usage, side effects, illness progression, diet/nutrition, medical adherence, and crisis anticipation/prevention
Maintain contact with clinical team members, patients’ other physicians/providers to coordinate optimal care and resources timely, per state regulations and company standards
Maintain patient medical records and documentation consistent with state regulations and company standards
Participate in continuing education as required by state
Prescribe medication as permitted by state prescribing authority
