Requirements

Registered nurse in the State with current licensure to practice professional nursing
Bachelor’s or master’s degree in nursing from an accredited program by the National League for Nursing preferred
Recent management and clinical experience a must
Demonstrated ability to supervise and direct professional personnel
Excellent observation, verbal, and written communication skills
Knowledge of HCHB tasks as it relates to quality and point of care documentation
Knowledge of State, CHAP, and Federal regulations in Hospice and/or Home Health

Responsibilities

Identifies and implements practices to improve quality and service
Establishes processes for proactive identification of issues and tracking of corrective action to minimize negative impact and maximize learning
Ensures the agency's Performance Improvement Program is comprehensive and in compliance with all licensing and regulatory requirements
Develops, monitors, and presents internal quality measures and initiatives
Provides education and support to the clinical staff under the direction of the Director of Education
Participates in the development, implementation, and evaluation of best practices for Home Health & Hospice
Supervises the Quality, Clinical Review, and Coding teams (Hospice)
Assists Clinical Managers in evaluating the quality of care provided by clinicians
Develops, implements, and collaborates on policies and procedures to ensure alignment with best practices and agency wide QAPI initiatives
Educates and trains individual branch QA staff
Collaborates with branch leadership on quality and process improvement
Facilitates learning experiences for staff through direct and indirect methods
