Requirements

  • Active, current, and unrestricted RN licensure in the state of residence (willing to apply for Compact RN, fees paid by company)
  • 5 years clinical practice experience as an RN
  • Associate Degree required (BSN preferred)
  • Ability to work Monday through Friday between 7:00am to 6:00pm CST (subject to change based on business needs)

Responsibilities

  • Telephonically assess, plan, implement, and coordinate case management activities with members to evaluate medical needs and facilitate overall wellness
  • Develop proactive course of action to address issues, enhance short- and long-term outcomes, and integrate opportunities for member wellness
  • Use clinical tools and data review to evaluate member needs, benefit plan eligibility, and facilitate transitions to programs and plans
  • Apply clinical judgment to incorporate strategies reducing risk factors, barriers, and addressing complex health/social indicators impacting care planning
  • Conduct assessments considering information from various sources, including co-morbid/multiple diagnoses, prior claims, work capacity, restrictions/limitations, and holistic referral needs
  • Consult with supervisor/others to overcome barriers, present cases at conferences for multidisciplinary focus
  • Utilize case management processes in compliance with regulatory and company policies/procedures
  • Use interviewing skills to maximize member engagement and discern health status/needs
