Requirements

  • Minimum 3+ years clinical practical experience as a Registered Nurse (RN)
  • Active and unrestricted Michigan RN licensure
  • Willing and able to travel 25-50% of the time in Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties
  • Reliable transportation (mileage reimbursed per company policy)
  • Proficiency with standard corporate software applications, including MS Office
  • Dedicated workspace free of interruptions for remote work
  • Separate care arrangements for dependents during work hours

Responsibilities

  • Develop a proactive plan of care to address identified issues and enhance short- and long-term outcomes and overall member wellness
  • Use clinical tools and information/data review to conduct an evaluation of member's needs and benefits
  • Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators impacting care planning
  • Conduct assessments considering information from various sources (e.g., claims) to address all conditions, including co-morbid and multiple diagnoses impacting functionality
  • Use a holistic approach to assess the need for referrals to clinical resources and other interdisciplinary team members
  • Collaborate with supervisor and key stakeholders in the member's healthcare to overcome barriers, meet goals and objectives, and present cases at interdisciplinary case conferences
  • Utilize case management processes in compliance with regulatory and company policies and procedures
  • Utilize motivational interviewing skills to ensure maximum member engagement and discern health status and needs
