Candidates need a four-year bachelor’s degree in a Health or Human Services related field with one year of experience working with Family Care target populations, or be a Wisconsin licensed Registered Nurse with one year of similar experience. Additionally, the selected individual must obtain Long Term Care Functional Screener (LTCFS) certification through required training.
This is a field position where core duties are performed at non-company locations like members' homes, primarily in Vernon County and surrounding areas including Viroqua, Westby, Coon Valley, De Soto, Chaseburg, and Ontario.
The typical schedule is Monday through Friday, 8:00 am to 4:30 pm CST, with up to 50% travel throughout Vernon County, WI and surrounding areas.
Being LTCFS Certified and having previous Family Care case management or screener experience are preferred.
