Responsibilities

  • Collaborate in a team-based model with lead care manager, community health worker, registered nurse care manager, and nurse practitioner to improve individuals' quality of life
  • Contribute behavioral health expertise to whole-person, interdisciplinary care for high-need patients
  • Participate in 1-2 monthly on-site visits in the community alongside team members (~10% of time; remainder remote)
