Requirements

  • Bilingual Licensed Clinical Social Worker (LCSW)
  • Ability to provide mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs
  • Experience or willingness to work in a team-based, interdisciplinary model with lead care managers, community health workers, registered nurses, and nurse practitioners
  • Comfortable with primarily remote work (approximately 90% from home), including 1-2 on-site community visits per month near your city

Responsibilities

  • Participate in 1-2 monthly on-site community visits alongside team members
