Responsibilities

Directly outreach and engage with individuals living with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or those with high medical needs
Build relationships and provide ongoing support to individuals eligible for Enhanced Care Management to improve their quality of life
Serve as an empathetic problem-solver, collaborating with partner clinics, community organizations, and Pair Team’s care coordinators
Manage six interdisciplinary pods, supporting up to 18 Lead Care Managers to drive high-quality patient care and effective engagement strategies for patients with complex medical and social needs
Lead and develop your team (build trust – as per incomplete section)
