Requirements

  • MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing
  • Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment (or equivalent combination of education and experience)
  • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW (as allowed by applicable state laws), or Clinical Psychologist to practice as a health professional within the scope of licensure in Virginia
  • Domiciled in Virginia (candidates residing in Norton, Bristol, Blacksburg, Roanoke, Danville, and Christiansburg encouraged)
  • Within reasonable commuting distance from posting location(s) in Southwest Virginia (unless accommodation granted)
  • For Government business: LAPC or LAMFT also acceptable (as allowed by state laws), plus any other state or federal requirements
  • For Puerto Rico associates (if applicable): Current PR health certificate and PR Law 300 certificate

Responsibilities

  • Perform case management telephonically and/or by home visits within scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs
  • Use appropriate screening criteria knowledge and clinical judgment to assess member needs
  • Conduct assessments to identify individual needs and develop care plan to address objectives and goals
  • Monitor and evaluate effectiveness of care plan and modify as needed
  • Support member access to appropriate quality and cost-effective care
  • Coordinate with internal and external resources to meet identified needs of members and collaborate with providers
  • Meet with members in person as per contractual requirements or as requested by the member
