Requirements

MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing
Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment (or equivalent combination of education and experience)
Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW (as allowed by applicable state laws), or Clinical Psychologist to practice as a health professional within the scope of licensure in Virginia
Domiciled in Virginia (candidates residing in Norton, Bristol, Blacksburg, Roanoke, Danville, and Christiansburg encouraged)
Within reasonable commuting distance from posting location(s) in Southwest Virginia (unless accommodation granted)
For Government business: LAPC or LAMFT also acceptable (as allowed by state laws), plus any other state or federal requirements
For Puerto Rico associates (if applicable): Current PR health certificate and PR Law 300 certificate

Responsibilities

Perform case management telephonically and/or by home visits within scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs
Use appropriate screening criteria knowledge and clinical judgment to assess member needs
Conduct assessments to identify individual needs and develop care plan to address objectives and goals
Monitor and evaluate effectiveness of care plan and modify as needed
Support member access to appropriate quality and cost-effective care
Coordinate with internal and external resources to meet identified needs of members and collaborate with providers
Meet with members in person as per contractual requirements or as requested by the member
