Requirements
- Enrollment in a Social Work program with required field education hours established by CSWE
- Functions under direct supervision of a licensed social worker
- Adheres to FMCNA Compliance Program, regulatory requirements, FMS policies, procedures, and training
- Complies with corporation, state, and federal agency regulations
- Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation
- Supports FMCNA mission, vision, core values, customer service philosophy, Quality Enhancement Program (QEP), and CQI activities
Responsibilities
- Supports FMCNA's mission, vision, core values, and customer service philosophy
- Drives FMS culture through values and customer service standards
- Provides outstanding customer service to external and internal customers
- Develops and maintains effective relationships through timely communication
- Responds, resolves, and follows up on customer service issues
- Assesses patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
- Participates in care planning with patient and interdisciplinary team to identify interventions for rehabilitation, treatment goals, and quality of life
- Utilizes FMS Ultra Care patient education programs, social work theory, methods, and quality of life instruments for assessment and care planning
- Provides psycho-educational counseling to improve treatment adherence, vocational/educational rehabilitation, and quality of life
- Offers supportive and goal-directed counseling for transplant-seeking patients
- Assesses patient awareness of advance directives, assists with forms/information, and facilitates discussions
- Provides general information about Do Not Resuscitate Orders and Advanced Directives
- Refers patient and team to community resources (e.g., home health, vocational rehabilitation)
- Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
- Assesses patient knowledge of kidney disease and barriers to treatment adherence
- Provides tailored education on kidney disease, working with patient, family, and healthcare team
- Assists in maintaining a safe and clean working environment
- Actively participates in process improvement activities related to patient satisfaction and QEP goals
