Responsibilities
- Coördineren van de zorg rondom bewoners
- Bewaken en verbeteren van de kwaliteit van zorg
- Optimaliseren van de afstemming met cliënten, familie en zorgprofessionals
- Afstemmen van complexe zorgvragen binnen het (multidisciplinair)team
- Signaleren van verbetermogelijkheden en vertalen deze naar concrete acties
- Fungeren als vraagbaak, coach en kwaliteitsbewaker binnen de afdeling
- Werken overstijgend en ondersteunen van het zorgteam waar nodig
