Responsibilities

  • Ambulante ondersteuning bieden aan cliënten thuis
  • Je inleven in de belevingswereld van de cliënt en zoeken naar de vraag achter de vraag
  • Positief en creatief kijken naar de krachten en mogelijkheden van de cliënt en oplossingsgericht werken
  • De cliënt volgen en coachen om zoveel mogelijk de regie over hun eigen leven te voeren
  • Werken vanuit een individueel ondersteuningsplan dat samen met de cliënt is opgesteld
  • Ondersteunen van cliënten met WLZ- of WMO-indicatie, met diverse ondersteuningsvragen
  • Werken aan zelfredzaamheid in het dagelijkse leven en terugval op steun van anderen
  • Inzetten op buurtpunten voor duurzame relaties, ontmoeting en verbinding (bijv. wijk-koken, buurtcirkels)
