- Master's degree in Social Work (MSW)

Responsibilities
- Assess patients' psychosocial status, strengths, and needs as part of comprehensive patient assessment
- Collaborate with patients and healthcare team to identify effective interventions for rehabilitation, treatment goals, and quality of life improvement
- Utilize patient education programs, social work theory/methods, interventions, and quality of life instruments in assessment and care planning to address barriers
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
- Assess patient knowledge of kidney disease and barriers to treatment adherence; provide supportive counseling within scope of practice
- Provide information and assist with referrals to community resources (e.g., home health, vocational rehabilitation) for optimal outcomes
- Assist patients with access to vocational rehabilitation to support employment or education
- Assess patient awareness of advance directives; assist with forms/information and facilitate discussions with team, patient, and family/support persons
- Participate in discussions of patient DNR status to ensure informed decisions
