Requirements
- Adheres to FMCNA Compliance Program, regulatory requirements, and FMS policy and procedure requirements

- Ability to assess patients' psychosocial status, strengths, and areas of need
- Knowledgeable of FMS Ultra Care patient education programs, quality of life measurement instruments, and FMCNA Social Work Policy, including documentation
- Ability to provide psycho-educational counseling, supportive and goal-directed counseling, and education tailored to patient's learning style and needs
- Familiarity with advance directives, Do Not Resuscitate Orders, and referral to community resources

Responsibilities
- Provides psychosocial services to in-center and home dialysis patients
- Works with the interdisciplinary health care team to promote positive adjustment, rehabilitation, and improved quality of life
- Informs, educates, and supports staff on the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families
- Participates in Quality Indicators, Outcomes, Quality Assessment and Improvement (QAI) activities, and process improvement to achieve FMCNA Quality Goals
- Drives FMS culture through values and customer service standards; develops and maintains effective relationships; resolves customer service issues timely
- Assesses patients' psychosocial status as part of comprehensive patient assessment and participates in care planning to identify interventions for rehabilitation goals and quality of life
- Provides outcomes-driven psycho-educational counseling to improve treatment adherence, vocational/educational rehabilitation, and quality of life
- Offers supportive counseling for transplant-seeking patients; assesses and facilitates advance directives discussions
- Provides information on Do Not Resuscitate Orders and Advanced Directives; assists with community resource referrals
- Participates in discussions of patient DNR status with physician, nurse, patient, and family
- Provides usual care and/or Social Work Intensive for non-adherence and quality of life concerns
- Assesses patient knowledge of kidney disease and barriers to treatment adherence; provides tailored education with patient, family, and health care team
