Requirements

  • Registered Nurse (RN) licensure
  • Ability to provide direct patient care involving heavy lifting (up to 30 lbs. as high as 5 feet), moving patients, assisting with ambulation, and occasionally moving machines/equipment up to 200 lbs. with assistance
  • Capability for frequent, prolonged standing/walking and bending over
  • Comfortable working in a health care facility environment with moderate noise, air temperature control, and potential exposure to infectious/contagious diseases/materials
  • Appropriate staff licensures and CPR certifications (implied for oversight role)

Responsibilities

  • Provide safe and effective oversight, support, training, and scheduling for home dialysis therapies in compliance with facility policies, procedures, and corporate/state/federal regulations
  • Manage profit and loss for the assigned program, optimizing operations to achieve/exceed budgets and key performance indicators
  • Direct and manage all aspects of patient care for Home Therapies patients from admission through discharge
  • Provide direction, guidance, and support to staff for safe and effective delivery of home therapy care to ESRD patients
  • Oversee patient and caregiver readiness, assess home environment suitability, and train on safe operation/preventative maintenance of equipment
  • Manage patient experience efforts, support satisfaction initiatives, embody company values, and encourage others to do the same
  • Ensure staff competency in home dialysis equipment operation, troubleshooting via telephone, and maintenance of licensures/CPR certifications
  • Manage program supplies and equipment tracking, ensuring timely replenishment of orders
  • Direct activities for Continuous Quality Assurance Improvement, patient retention, Clinical Algorithm Adoption, patient access management, and training success
  • Direct data entry for patient admission, regulatory reporting, billing, physician orders, lab results, vital signs, patient status, and treatment data
  • Ensure timely completion of regulatory, compliance, and audit activities
  • Orient and support new staff, provide ongoing education, and ensure compliance with risk management initiatives (including in-center staff)
  • Participate in Business Unit initiatives, projects, and audits as directed
  • Perform other related duties as assigned
