Position Overview

This is a full-time, telework role.

Working Schedule: Monday-Friday, standard business hours, including 2 evening shifts per week from 11:30 am - 8:00 pm CST.

The primary focus of this position is to educate members with diabetes and other conditions, provide them with resources, and assist in closing gaps in their care. The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and promote member decisions related to achieving and maintaining optimal health status.

Key Responsibilities

Assessment

  • Conduct comprehensive evaluations of member needs and benefit plan eligibility for integrated programs and services using clinical tools and data review.
  • Utilize assessment techniques to determine member's level of health literacy, technology capabilities, and readiness to change.

Enhancement of Medical Appropriateness & Quality of Care

  • Assess benefits and member needs to ensure appropriate utilization of services and integration with internal/external programs, applying relevant criteria, guidelines, health management plans, policies, procedures, and regulatory standards.
  • Employ a holistic approach, consulting with supervisors, Medical Directors, and others to overcome barriers to achieving goals.
  • Identify and escalate quality of care issues through established channels.
  • Utilize influencing and motivational interviewing skills to maximize member engagement and promote lifestyle/behavior changes for optimal health.
  • Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices.
  • Interpret and utilize clinical guidelines/criteria to positively impact members' health.
  • Provide up-to-date healthcare information to facilitate member understanding of their health status.
  • Help members actively and knowledgeably participate with their providers in healthcare decision-making.

Monitoring, Evaluation, and Documentation of Care

  • Develop and monitor established plans of care in collaboration with the member and/or attending physician to meet member goals.
  • Utilize internal policies and procedures in compliance with regulatory and accreditation guidelines.

Requirements

  • Registered Nurse (RN) with an unrestricted license in their state of residence, holding multistate/compact privileges, and the ability to be licensed in all non-compact states.
  • 3+ years of clinical nursing experience post licensure.
  • At least 1 year of experience working with patients with diabetes.
  • Must possess or be willing and able to obtain high-speed internet access.
  • Must be willing to obtain certification as a Certified Diabetes Care and Education Specialist (CDCES).

Preferred Qualifications

  • Certified Diabetes Care and Education Specialist (CDCES).
  • Previous coaching experience.
  • Managed Care Organization (MCO) experience.
  • Experience with Microsoft Word, Outlook, and Excel; comfortable using various computer programs.

Education

  • Associate's degree minimum required as a Registered Nurse.
