Requirements

Licensed as LCSW, MFT, or LPCC (bilingual capability)
Ability to provide mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs
Willingness to conduct 1-2 on-site community visits per month alongside team members (near your city), with ~90% remote work
Ability to work in a team-based, interdisciplinary model with lead care managers, community health workers, registered nurses, and nurse practitioners
Highly motivated, creative, and empathetic mindset to support whole-person care

Responsibilities

Primarily work with and support enrolled individuals struggling with severe mental illness or substance use disorders
Use evidence-based approaches to provide support, mental health resources, and therapeutic interventions
Contribute expertise to improve individuals' quality of life through behavioral interventions in a collaborative care team
Participate in 1-2 monthly in-person community visits (close to your city) while performing majority of duties remotely
