Requirements

  • Licensed as LCSW, MFT, or LPCC (bilingual capability implied by job title)
  • Ability to work primarily remotely with 1-2 on-site community visits per month near your city (90% remote)
  • Expertise in providing mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs
  • Willingness to work in a team-based, interdisciplinary model with lead care managers, community health workers, registered nurses, and nurse practitioners
  • Highly motivated, creative, and empathetic mindset to support whole-person care

Responsibilities

  • Primarily work with and support enrolled individuals struggling with severe mental illness or substance use disorders
  • Use evidence-based approaches to provide support, mental health resources, and therapeutic interventions
  • Contribute behavioral health expertise to improve individuals' quality of life in a collaborative care team model
  • Participate in 1-2 monthly on-site community visits alongside team members (approximately 10% of duties)
