Responsibilities

Primarily work with and support enrolled individuals struggling with severe mental illness or substance use disorders
Use evidence-based approaches to provide support, mental health resources, and therapeutic interventions
Contribute expertise to an interdisciplinary team (with lead care manager, community health worker, RN care manager, and NP) to improve individuals' quality of life
Participate in 1-2 monthly on-site community visits alongside team members (close to your city)
Engage in remote work for approximately 90% of duties from home
