Requirements

  • Licensed as LCSW, MFT, or LPCC (bilingual preferred, implied by job title)
  • Ability to work primarily remotely with 1-2 on-site community visits per month near your city (90% remote)
  • Expertise in providing mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and/or high medical needs
  • Proficiency in evidence-based behavioral interventions
  • Ability to work collaboratively in an interdisciplinary team-based model (with lead care manager, community health worker, registered nurse care manager, and nurse practitioner)

Responsibilities

  • Primarily work with and support enrolled individuals who struggle with severe mental illness or substance use disorders
  • Use evidence-based approaches to provide support, mental health resources, and therapeutic interventions
  • Contribute behavioral health expertise to improve individuals' quality of life in a whole-person, interdisciplinary care model
  • Engage in 1-2 on-site community visits per month alongside team members (approximately 10% of duties)
  • Collaborate with lead care manager, community health worker, registered nurse care manager, and nurse practitioner to deliver compassionate, high-impact care
