Candidates must possess a valid Registered Nurse (RN) license, and a minimum of three years of experience working with individuals who have chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
Candidates should possess a high school diploma or equivalent, and demonstrate a minimum of one year of experience in a social services or case management role, with a preference for experience working with individuals with intellectual and developmental disabilities. Strong communication and interpersonal skills are essential, along with the ability to conduct assessments and develop individualized support plans.
